Old Southwest Rehabilitation and Nursing is a 130-bed Skilled Nursing Facility (SNF) and Intermediate Care Facility (ICF), providing 24-hour nursing care. Services can include wound care, IV infusion therapy, pain management, restorative nursing care, stroke recovery program, trach management and support, and more. Provides counseling and discharge planning.
Provides respite care services. The facility does accept Alzheimer's patients on a case-by-case basis, but does not have a designated Alzheimer's unit. Will take multiple family members.
